Teaching and Learning Forms
Forms can be submitted through 2 channels:
- Student Affairs and Alumni Office, Faculty of Medicine
- Submit the file via email: medafpsu@gmail.com
- Download the form (.pdf file).
- Fill in all details completely, clearly, and accurately, and attach relevant documents, such as a medical certificate in case of sick leave.
- Contact the course chairperson to review and sign the leave request form for approval.
- Submit the approved leave request form, signed by the course chairperson, to the Student Affairs and Alumni Office through 2 channels:

- Submit the form at least 1 month in advance.
- Submit the form to the Student Affairs and Alumni Office, 7th Floor, or via email: medafpsu@gmail.com
- This form is for students who are unable to pay tuition fees by the university’s deadline, including students applying for student loans and students awaiting scholarship consideration.
- The process should be completed at least 7 days before the registration deadline.
Scholarship Forms

This form is for undergraduate students of the Faculty of Medicine, Prince of Songkla University, Academic Year 2025, who received a scholarship in Academic Year 2024.
This form is an attachment for transferring funds to a Siam Commercial Bank account, Prince of Songkla University Branch. The account name must be the student’s name.
